DVT & Cellulitis

 

Cc: right leg swelling 

Diagnosis :

DVT

Differential diagnosis:

Baker’s cyst 

Cellulitis 

Knee Trauma 

Joint problem

Examination:

Calf swelling ( use tape measure , mark 10 cm below tibial tuberosity ), positive if > 3 cm difference 

Look for superficial venous engorgement & pitting edema 

Check peripheral pulses

What will you do ?

D. Dimer 

Do U/S doppler leg

Management of DVT:

Anticoagulation : 3 month if provoke , 6 months if unprovoked, lifelong if high risk or recurrent 

Compression stockings

Complications of DVT: Pulmonary embolism

• Ask for sob or chest pain 

• Use Wells score

Options for cellulitis:

Clindamycin 

Ceftriaxone (Rocephin ) 

Trimethoprim-sulfamethoxazole ( septrin ) 

Vancomycin or Linozolid 

Penicillin

Cefazolin
