CC: cough

Diagnosis :

Bronchiectasis

Clinical signs:

Clubbing

Coarse cough all over chest, changing with cough

Other signs like connected to oxygen via face mask / nasal prong

Area to examine:

Chest

Extra: lymph nodes & listen for loud P2

Differential diagnosis:

Pulmonary fibrosis 

Cancer

Advanced sarcoidosis

What will you do ?

Basic profile like CBC, LFT, …

CXR

HRCT chest

Investigate the causes example Sweat chloride and genetic test for cystic fibrosis, …

How you differentiate between pulmonary fibrosis & bronchiectasis ?

Crackles usually heard basal in pulmonary fibrosis while in bronchiectasis is all over the chest 

Management of bronchiectasis:

Physiotherapy

Antibiotic for exacerbation

Long-term low dose azithromycin

Bronchodilator and inhaled steroid

Surgery

Complications of bronchiectasis:

Hymoptysis ( mycotic aneurysm )

Amyloidosis ( do urine for protein )

Cor pulmonale ( loud p2 )
