SpO2 100% on room air
Investigations
Blood lactate level
CBC and RFT
ABG: pH 7.19, PCO2 39, HCO3 8
COHb level
CXR to look for pulmonary edema
ECG to rule out myocardial infarction
High lactate: 10 then 8 after IVF replacement
CT brain: bilateral symmetrical hypoattenuation of globi pallidi due to necrosis
CK to rule out rhabdomyolysis
What Will You Do?
Call ICU for assessment
ENT consult for bronchoscopy
Give IV fluids
Give Losec IV
Give oxygen by rebreather mask at 15 L/minute
Consider Hyperbaric Oxygen?
For severe CO poisoning:
Loss of consciousness
Pregnancy
Persistent neurological deficits
Cardiac ischemia
Severity
Carboxyhemoglobin level > 25%
Complication
Movement problems (Parkinson’s disease-like syndrome)
