Description

Clinical Presentation
  • Multiple patients arrived in resuscitation
  • Low blood pressure
  • Nausea and vomiting
  • GCS 15/15
  • BP 80/50
  • SpO2 100% on room air
Investigations
  • Blood lactate level
  • CBC and RFT
  • ABG: pH 7.19, PCO2 39, HCO3 8
  • COHb level
  • CXR to look for pulmonary edema
  • ECG to rule out myocardial infarction
  • High lactate: 10 then 8 after IVF replacement
  • CT brain: bilateral symmetrical hypoattenuation of globi pallidi due to necrosis
  • CK to rule out rhabdomyolysis
What Will You Do?
  • Call ICU for assessment
  • ENT consult for bronchoscopy
  • Give IV fluids
  • Give Losec IV
  • Give oxygen by rebreather mask at 15 L/minute
Consider Hyperbaric Oxygen?

For severe CO poisoning:

  • Loss of consciousness
  • Pregnancy
  • Persistent neurological deficits
  • Cardiac ischemia
Severity
  • Carboxyhemoglobin level > 25%
Complication
  • Movement problems (Parkinson’s disease-like syndrome)
