Differential Diagnosis
  • Knee trauma
  • Baker’s cyst
  • Joint problem
  • Cellulitis
Examination
  • Check peripheral pulses
  • Look for superficial venous engorgement and pitting edema
  • Measure calf swelling using tape measure 10 cm below tibial tuberosity
    Positive if > 3 cm difference
What Will You Do?
  • D-Dimer
  • Ultrasound Doppler leg
Management of DVT
  • Anticoagulation:
    • 3 months if provoked
    • 6 months if unprovoked
    • Lifelong if high risk or recurrent
  • Compression stockings
Complications of DVT
  • Pulmonary embolism

Ask for:

  • SOB
  • Chest pain

Use Wells score

Options for Cellulitis
  • Clindamycin
  • Penicillin
  • Trimethoprim-sulfamethoxazole (Septrin)
  • Ceftriaxone (Rocephin)
  • Cefazolin
  • Vancomycin or Linezolid
